I understand that I am responsible for any Library card issued to me that is associated with my membership with the Theosophical Society in America and/or the Olcott Memorial Library, and all materials checked out on this card until such time that this card is reported to Library staff as lost or stolen.

I am further responsible for any charges that may result from late return, loss, or damage of materials borrowed through my card.

Summary of Fees

* To ensure the timely availability of material to all our patrons, late fees are assessed on all past-due materials.
** A processing fee of $10 per title will be charged for any materials lost or damaged while in the patron’s possession. Additionally, the patron is responsible for the item’s replacement cost, plus applicable overdue fines.
